(Note: This form must be completely filled out (except for IV) and mailed back, along with the candidate's statement and resume, to the Chicago Office no later than 21 days prior to the Spring Conference, by first-class certified mail, return receipt requested, or overnight mail (Federal Express, Cannon Ball, etc.), in order for a student to be eligible for certification as a nominee for national office.)
